Responsibilities:
Provides, coordinates, and facilitates patient care in the following areas: discharge planning, swing bed, utilization review and ACO activities in the hospital setting. Assists the hospital team in coordinating patient care from admission throughout the hospital stay, discharge and continues after discharge. Assists with organizing services across provider lines between team members and systems to affect optimal patient outcome, achieve continuity of care, reduce costs, and reduce readmissions. Serve as a utilization management resource to determine appropriateness of admission, level of care and continued stay.
Qualifications:
4 years’ clinical experience required. Prior case management and utilization review experience including knowledge of MCG and/or InterQual preferred. Highly motivated with excellent oral and written communication skills. BLS certification required.
For RN applicant: Current Illinois RN license. BSN preferred.
For Social Worker applicant: Current Illinois LCSW license. MSW required

Performs a variety of Information Systems (IS) development, maintenance and support functions with minimal supervision. Responsibilities may include but not limited to: design and development of forms and reports for software applications and website, administration and technical support of the hospital’s computer network and client-server software, preventive maintenance, cabling (install and removal), analysis of both network and electrical systems, as well as providing technical support and coordination of IS training and presentation resources.

Job Duties:
The ED RN ensures the delivery of and provides quality nursing care for patients of all ages presenting to the Emergency Department. The RN works collaboratively within a multidisciplinary healthcare team.
The RN will incorporate caring for all patients in all settings, and includes, but is not limited to, all of the following: A comprehensive nursing assessment, the development of a plan of nursing care to be integrated within the patient centered health care plan, and setting goals to meet identified health care needs, determining nursing interventions, and implementation of nursing care through the execution of nursing strategies and regimens ordered or prescribed by authorized healthcare professionals. The Registered Nurse will administer medication in a timely manner. The nurse will delegate duties to comply with the patient’s current plan of care and advocate for patients. The Registered Nurse will evaluate responses to interventions and the effectiveness of the plan of care. The nurse must communicate and collaborate with other health care professionals as needed to ensure continuity of care.
